Care Management

An expert to take stock and build the plan.

What it is

A care manager, sometimes called an Aging Life Care professional, is the expert who looks at the whole picture: your loved one's health, home, finances, and wishes, and turns it into a clear plan. Some families want this first. Others bring one in when things get complicated.

When to consider one

  • You are managing a loved one's care from a distance.
  • There are several health conditions to juggle at once.
  • The family does not agree on what to do next.
  • A hospital is discharging your loved one and there is no plan in place.
  • You want a roadmap before a crisis, not during one.

What to expect

  • A full assessment of the situation.
  • A written plan you can act on now or keep ready for later.
  • Someone to adjust the plan as things change.

Where it can lead

A care manager may recommend staying at home, or that it is time to move. There is no single path, and you can move sideways as the situation changes.
